Short answer: no. You cannot tell whether sperm is healthy by looking at semen. Colour, thickness and volume describe the seminal fluid — the sperm cells themselves are microscopic and effectively transparent. The three parameters that determine fertility (concentration, motility and morphology) all require magnification to measure.
Visual signs can flag infection or blood, which is useful. They cannot estimate your chance of conceiving. Only a semen analysis can do that.
This is the question underneath most searches about semen appearance. People aren't really curious about pigment chemistry they want to know whether what they're seeing means they can father a child.
The honest answer is uncomfortable but useful, and it saves a lot of wasted worry: you're looking at the wrong thing.
Why looking doesn't work
Semen is roughly 95–98% seminal plasma fluid secreted by the seminal vesicles, prostate and bulbourethral glands. The sperm cells occupy the remaining 2–5% of the volume. Each one is about 50 micrometres long, around a twentieth of a millimetre, and individually transparent.
So when you assess semen visually, you are assessing accessory gland secretions. You are not seeing sperm. This is why a man with azoospermia — no measurable sperm at all — produces ejaculate that looks entirely normal to the naked eye.
The WHO laboratory manual for the examination and processing of human semen (6th edition) reflects this in how it structures assessment. Colour, volume, viscosity and liquefaction sit under macroscopic examination — a screening step. Concentration, motility and morphology sit under microscopic examination, and those are the parameters with reference limits tied to time-to-pregnancy data. What are signs of unhealthy sperm?
The framing of this question is part of the problem. "Unhealthy sperm" is a microscopic finding, and microscopic findings have no reliable outward signs. Most men with significantly impaired semen parameters have no symptoms whatsoever — which is precisely why male factor infertility is so often discovered late, after a couple has already been trying for a year or more.
That said, there are indirect signals worth knowing about. They don't diagnose anything, but they justify getting tested.

Anabolic steroids deserve a specific mention because the pattern is so common and so poorly understood: exogenous testosterone suppresses the hormonal signal that drives sperm production, and can cause azoospermia in men who feel, and look, perfectly healthy. How to tell if a guy has healthy sperm
You can't, without testing. A semen analysis measures exactly this, and the list of what it covers is short.

Two caveats that matter more than most people realise.
These are reference limits, not pass/fail lines. They represent the 5th percentile of men who conceived naturally within 12 months. Falling below one doesn't mean you can't conceive; sitting above them all doesn't guarantee you will.
Results fluctuate substantially. An illness three months ago, a run of poor sleep, heat exposure or a period of high stress all shift the numbers. Sperm production takes about 74 days, so today's sample reflects the last two to three months of your life. This natural variation is why clinicians rarely act on a single result, and why tracking over time is more informative than one snapshot.
Does thick sperm mean you're fertile?
No. There is no relationship between semen thickness and fertility — and if anything, the association runs the wrong way.
Thick semen at ejaculation is normal for everyone. It's caused by semenogelin from the seminal vesicles, which coagulates the ejaculate into a gel, and it liquefies within 15–30 minutes as prostatic PSA breaks it down. Every healthy man's ejaculate does this.
Semen that stays thick after liquefaction should have completed is hyperviscosity, and that is associated with reduced fertility, not increased. Du Plessis et al. (2013) found it in 12–29% of ejaculates and linked it to male factor infertility in both natural conception and IVF; Barbagallo et al. (2021) found elevated oxidative stress in hyperviscous samples.

Does thick sperm mean high testosterone?
No. Semen texture is not a testosterone marker, and this myth gets the physiology backwards.
Testosterone does influence the accessory glands — the seminal vesicles and prostate are androgen-dependent, so low testosterone tends to reduce their secretory output, which lowers ejaculate volume. Lower volume can make a sample look thicker and more concentrated. If there's any relationship at all between thick-looking semen and testosterone, it points down, not up.
More importantly, high testosterone is not a marker of good sperm production either. The testes need a very high intratesticular testosterone concentration to make sperm, and that is driven by luteinising hormone from the pituitary. Taking external testosterone raises blood levels while suppressing the pituitary signal, collapsing intratesticular testosterone and shutting down spermatogenesis. Men on TRT frequently have excellent blood testosterone and no sperm at all.

Can yellow sperm get pregnant?
Yes, usually. Yellow semen is most often caused by concentrated urine in the urethra, dehydration, a long abstinence period, or B vitamins — none of which affect sperm function.
The exception is when yellow shades into yellow-green and reflects infection. Then it can matter, for two reasons:
- Oxidative damage. High white blood cell counts in semen raise reactive oxygen species. A 2020 review covers how leukocytospermia damages sperm membranes and DNA. Elevated sperm DNA fragmentation can reduce fertilisation and increase miscarriage risk even when standard parameters look normal.
- Obstruction. Untreated infection can scar the epididymis or vas deferens.
Practically: if the semen is yellow but there are no other symptoms, it isn't a barrier to conception. Yellow-green with pain or burning should be treated first. Can watery sperm get someone pregnant?
Yes — watery semen frequently results in pregnancy.
The most common cause of watery semen is frequent ejaculation, which doesn't reduce fertility in any meaningful way. In fact, for couples trying to conceive, having sex every one to two days across the fertile window generally produces better outcomes than saving up — regular ejaculation reduces the time sperm spend in the epididymis and is associated with lower DNA fragmentation.
Where watery semen matters is when it's persistent and accompanied by low volume. That combination can reflect:
- Genuinely low sperm concentration (oligozoospermia)
- Retrograde ejaculation — semen entering the bladder; the tell is cloudy urine afterwards
- Low testosterone or a pituitary issue
- Partial obstruction of the ejaculatory ducts

What visual assessment is genuinely good for:
- Spotting blood, which needs checking
- Flagging possible infection, which needs treating
- Noticing persistent low volume or delayed liquefaction, which justify a test

- Assess how well they swim
- Assess their shape
- Assess DNA integrity
